In the official Aniimo index the line runs Susuta (Basic Form) → Popota (Lumin Stage) → Panpanta or Piopiota (Gamma Stage). Where two names share a stage, the line branches: the stage before evolves into one or the other. The index does not say what triggers each step or which branch you get — we will add that from our own playthrough rather than guess.

How many forms does Panpanta have?
